Introduction
This efolio is a presentation of my inquiry into Crew Resource Management (CRM), specifically the communication issues between flight crew and the cockpit, how those issues impact flight safety and how they can lead to accidents.
CRM training addresses the skills and attitudes of those involved in aviation "including communications, situational awareness, problem solving, decision making, and teamwork; together with all the attendant sub-disciplines which each of these areas entails" (Skybrary, 2014). The consensus gleaned from numerous NTSB reports as well as in depth scholarly research is that 80% of airplane accidents are the result of human error and, more specifically pilot error. Although efforts have been underway to differentiate the elements of pilot error (Shappell & Wiegmann, 2000; Wiegmann & Shappell, 2001), from the various related causes including interpersonal communications errors, it is not an exact science. It is the intention of this writer to focus on those causes.
